If you wanted native support, you could buy the smart card reader + cable + HDD cover and add it in yourself. Then you could add the USB 3 card as well. If I needed a smart card reader, I'd go that route - the ExpressCard34 smart card readers I've seen either stick out from the laptop or require you to "unfold" part of it to get the smart card in.

I bought a T420 (type 4178) which comes without integrated smart card reader device. However additionally I bought smart card reader (FRU 04W1638) and a cable for the reader (FRU 04W1639) which I installed and it works perfectly. The smart card reader for my T510 arrived today and its installation was identical to that of the T520. There are a couple of different FRU numbers for these readers but they all install into the W/T510/520 chassis and are recognised without problem.
The Smart Card reader is entirely different than the SD card reader. I've not had a thinkpad w/ smart card reader as my primary machine, although I've always been intrigued to toy with it for logon.
